EYELASH EXTENSION LIABILITY WAIVER
By law, we are required to have an intake form in our records, for each service, once per calendar year.
We have provided these forms for your convenience, as some people like to complete this prior to coming in for their appointment. However, we will gladly provide you with a copy of the necessary form at the time of your appointment. Please allow time to complete this if this is your first time having a service performed at epic, so far this current calendar year.
